Step 1: Identify the Record

Describe the record and the right you wish to exercise or protect.

Step 2: Complete Form 02

Download Form 02 from the Information Regulator

Step 3: Submit Your Request

Step 4: Fees

Request and access fees may apply. HWK will notify you of any fees before processing.

Step 5: Timelines

HWK will respond within 30 calendar days unless an extension is permitted under PAIA.

8. Grounds for Refusal & Remedies

HWK may refuse access where disclosure would:

Unreasonably reveal personal information of a third party
Reveal confidential or commercial information
Endanger life or physical safety
Prejudice security of property
Be privileged in legal proceedings
Prejudice research

Remedies

Lodge a complaint with the Information Regulator
Apply to a court with jurisdiction
